What the contact form collects
Your name, how you want us to reach you, your phone number or email, and anything else you choose to fill in, such as your ZIP code, your child's age range, your insurance carrier's name and a short note.
What we ask you not to send
Please do not send diagnoses, member ID numbers, health records, your child's name or date of birth. We will tell you how to send records securely after we talk.
Who sees it
Only the Puzzle Toys staff who reply to requests. The owner and legal counsel will confirm how requests are stored and delivered before this site goes live.
How long we keep it
We keep a request only as long as we need it to reply and to follow the law. The exact period will be set before launch.
Asking us to delete it
Call or email us and we will delete your request, unless the law requires us to keep it.
Cookies and tracking
This site sets no advertising or tracking cookies, loads no third-party scripts, and uses no analytics. It saves your display choices (motion and colors) in your browser's local storage. That stays on your device.
